Hello and welcome back to SAU! If your admissions rep asked you to fill this out, then you are returning as a student and you have an SAU account that was disabled in the past. SAU's Information Technology team can work to recreate your account, but the process is not automated. Please choose the "request service" option to provide the Information Technology team what we need to know to get your account activated. We will reach out to you with the next steps once we link and recreate your account.

The Documentation Request form determines the necessity and prioritizes the creation of documentation for the eLearning team, subject matter experts, lead faculty, instructors, and students.
